choices to our consumers.

Adult Day Services
These are services or supports provided away from the person's home, typically during working hours, to increase the person's independence, integration, inclusion, and personal accomplishment. To the extent possible and desired by the person and guardian/family, typical work or job related activities are addressed during the time. Choices include:
* Sheltered Workshop
* Supported Employment
* Alternative Choice Program
Adult Residential Services
Through our affiliates, these services and supports are provided to assist the person to live successfully in a community setting they have chosen and can afford.
Family Support Services
These are services which support the family in effectively and successfully raising and keeping a person with developmental disabilities in the family home.  Family Support Services are available from several resources, including the Home and Community Based Services Waiver and Family Services/Supports grant.
Service Coordination
This is a service in which help is given to persons with developmental disabilities and their guardians/families to obtain and maintain needed services, supports, and funding through one or more of the following tasks:
  • assessment of needed supports and services
  • support planning
  • coordination of services
  • advocacy
  • assisting with life transitions and portability of services
  • Eligibility and Funding

     In general, people who meet the developmental disabilities definition who wish to receive services in Lyon, Morris, Chase, and Wabaunsee counties are eligible for services. Eligibility does not guarantee immediate access to services. Access to some services and supports is at least partially dependent on funding availability. Service coordinators will help people determine if funding is available for needed services and assist them in obtaining those services.
